General Data Protection Regulation - Privacy Policy
This page informs you of what, why and how the personal information I collect about you is used, stored and disposed of.
What: Personal details i.e. name, address, DOB, contact details,GP details. Private therapeutic information i.e. contract/agreement, assessment information, background information, GDPR agreement.
Why: Personal details are held in case I need to contact you or contact your GP (with consent unless under exceptional circumstances, such as you are at risk to yourself or others). Private therapeutic information includes brief session case notes, recording work completed or to be completed.
How used: Personal details i.e. telephone number will be kept on my mobile phone in case I need to contact you when I am away from my office, your e-mail address will be kept on my contacts on my PC. These will be used to contact you directly. Both my phone and PC are password protected.
How shared: In order to maintain confidentiality on your behalf I will not share your personal information with anyone else without your consent. In exceptional circumstances I may need to break this confidentiality, if I feel you are at risk of harming yourself or others or if I am required by a court or legal authority. I participate in monthly supervision sessions for my professional practice. In order to protect your privacy, within these sessions any information verbally given will be anonymised so that you the client cannot be identified and will only be referred when helpful to my professional practice.
How stored: All client information is stored in a locked filing cabinet, personal contact details are kept separate from private session case notes. The session case notes are identified by a case reference number so that they are not associated with personal details. All e-mails or other electronic documentation are not retained on my PC they are binned and cleared regularly.
How disposed of: In case you choose to return to me for counselling and also in accordance with my professional body and insurance I will keep modified client records for 7 years after our work is complete. After this time has passed I will shred all your client records. Contact details on my phone and PC are deleted once therapy work has ended.
Having read and signed the GDPR agreement you will have given your clear consent for me to collect your personal information for the specific purpose of therapy.
You have the following rights:
· To be informed what information I hold
· To see the information I hold about you
· To rectify inaccurate, out of date or incomplete personal information
· To request your personal information to be erased where it is no longer necessary for me to retain this information
· To withdraw your consent for me to use your personal information.
